“My Growth” Handouts
You will receive these handouts at each well child visit. Their purpose is help you to follow your child’s growth and development. Each handout has a section on appropriate developmental milestones for that age level. There is also a box where we will write down your child’s measurements and percentiles for growth. Additionally, these forms provide anticipatory guidance with regards to safety issues and routine care. Feel free to download and print the sheets prior to your visit so that you can review what issues will be addressed at your next visit.
