Benefits and Covered Services
As outlined below, we accept most major insurance carriers. That being said, insurance carriers often have many smaller group plans, for which we have no control over participation in those plans. While we accept the main insurance, we may not be on your specific plan as in network. It is your responsibility to always check with your insurance plan to make sure our providers are in your network BEFORE scheduling a visit. If your insurance plan changes, be sure to check again with the new plan as coverage could change. We make every effort to verify coverage, but this does not necessarily guarantee that we are in network. You will be responsible for any charges that are not covered by your insurance plan.
Self Pay
If you are uninsured or choose to not file with your insurance company, you will be responsible for the balance for your visit at the time of service.

Some helpful hints for your visit:
- Always bring your insurance card to the appointment
- Know your benefits and the amount of your copay
- Know who your primary care provider is
- Make sure you are within the effective date of the policy
- Know if you need a referral from the primary care physician to see a specialist, for xrays, etc.
